HIV Case Manager and Prevention
The HIV Case Manager assures the provision of client-centered services in line with the client’s treatment plan and HIV Continuum of Care, including linking clients to healthcare, psychosocial, and other services for persons living with HIV/AIDS and those at risk for HIV/AIDS. The HIV Case Manager ensures timely and coordinated access to medically-appropriate levels of health and support services as well as continuity of care through ongoing assessment of client’s needs and personal support systems.
Community Prevention Specialist
The Community Prevention Specialist will assist with planning, developing, implementing, and administering substance abuse and suicide prevention services and activities based on community health needs. Works with local, state, and federal agencies as well as private and community entities in coordination of prevention activities. Assures compliance with all local, state, federal program requirements and regulations.
The Nurse Practitioner who will ensure the delivery of essential public health services and client services through programs that include immunizations, Title X family planning, sexually-transmitted disease clinic, and epidemiologic disease follow-up. Our health department serves the Natrona County community by promoting healthy behaviors and equal access to quality health care and serves as a resource for current and reliable health information. Work activities will focus on the provision of women’s health services under the Title X Family Planning program.
