The Public Health Nurse Case Manager functions as a member of the Adult Health Team. A primary duty of this job, as a public health nurse would include functional assessments (LT-101) in the community. Provide Medical Case Management (MCM) services including client centered services that implement the clinical treatment plan, including linking clients with health care, psychosocial, and other services. A nurse hired for this position is expected to participate in development and implementation of new projects such as Fall prevention and Medicare Case Management Services. CNCHD expects the nurse to participate and collaborate with other community organizations serving the same client population. This position may participate in epidemiological (vaccine preventable) investigations, and participate in immunization clinics. The employee in this position will become competent in public health emergency preparedness activities.
Maternal Child Health Medical Case Management (MCM) services include client centered services that implement the clinical treatment plan, including linking clients with health care, psychosocial, and other services. The MCM ensures timely and coordinated access to medically appropriate levels of health and support services as well continuity of care through ongoing assessment of the client’s needs and personal support systems. Clients are seen in their home or CNCHD office.
