An LT101 is a functional assessment used as a screening tool to determine whether an individual requires a nursing home level of care. The assessment is also used to determine eligibility for Wyoming Medicaid long-term care programs and services. The LT101 evaluates an individual’s ability to function independently and measures how much assistance is needed to care for themselves.
