The Casper-Natrona County Health Department provides a variety of community-based case management  and other community services.  These programs aim to improve individuals’ health outcomes without over-extending the community’s health care system.

Adult Health

The Adult Health program at the Casper-Natrona County Health Department (CNCHD) provides senior and disabled adults the ability to remain in the home and to ensure an optimal level of functioning. Our public health nursing staff visit with the clients in their home setting and provide case management of chronic diseases and skilled nursing services. Our public health nurses provide education, health assessments, case management and monitoring of medication regimen and compliance.

LT101

LT101 Assessment is an assessment developed and used by the Wyoming Department of Health, to determine whether an individual requires, or continues to require the services or level of care provided in a nursing facility. The assessment is used in the consideration of eligibility for certain Wyoming Medicaid long-term care programs and services. The LT101 is designed to evaluate an individual’s ability to function independently and measure how much assistance is needed to care for themselves.
